Why I collect this information
To plan and provide a diagnostic assessment which the client has requested
How I will store the data
All data will be stored securely at my address and where available digitally, will be password protected.
Reports will be sent to the client by email as a password protected PDF. This is a confidential document.
How long I will keep this information
Following the assessment and the compilation of the final assessment report, all client questionnaires, raw data test sheets and all other personal data will be permanently deleted/destroyed.
The final assessment report will be held by the assessor for 6 years after the client’s 18th birthday or for 6 years for an adult. During this time, you will be able to ask for an electronic copy of the report. However, I would strongly recommend that you keep a copy of your report securely. After this time, I will not be able to supply you with a copy.
Who might I share this information with?
I will not share this information without permission from the client.
Occasionally, I may share sections of a report or a whole report that has been anonymised (with any identifying information removed) amongst professionals, for the purpose of appraisal, evaluation, sharing good practice or training.
I may share anonymised reports with the Professional Association of Teachers Of Students with Specific learning difficulties (PATOSS) for their evaluation and assessment.
Access to your personal information
Under GDPR, the data subject has the right to access and amend any of their personal data that I hold.
You are entitled to view, amend, or delete the personal information that I hold. Email your request to info@surreyhillsdyslexia.co.uk
Note: Requests from clients to delete data relating to an individual will be considered on a case-by-case basis but I may be unable to remove all assessment records for legal reasons.
